NO MACROS FOR TEACHING PHYSICIAN STATEMENT*
*Exception: See our new tip when using electronic medical records.
- Signature Stamps - a facsimile of your signature is not permitted. Neither is cutting and pasting a scanned image of your signature to the medical record. You may use a stamp with your printed name and provider number to help identify your signature. ESA (Electronic Signature Authorization) is permitted, but passwords for the electronic systems must not be shared and the responsibility of authenticating reports must not be delegated to anyone.

- CMS has confirmed that NO MACROS are permitted in generating the Teaching Physician Statement.*
o The Teaching Physician must PERSONALLY provide his or her own statement indicating he or she has seen and evaluated the patient and reviewed the Resident's note.
o The Resident may not write or dictate the Teaching Physician note
o You may not direct transcription to "use the Teaching Physician statement", or any other electronic shortcut, such as cutting and pasting or a macro.
- When a physician and an ARNP/PA combine an inpatient services or services in a hospital based clinic - the Teaching Physician must make a brief statement confirming face-to-face contact with the patient.
For example: "I have seen and evaluated this patient, see today's ARNP note."Do not use fill in stamps, preprinted statements, or dictation macros for the combining note. Scribes only write what another does - term does not apply when person writing also performs professional service - Use of scribes is strongly discouraged and only when the term accurately applies.
