Referrals and Consultations.
The Neuro-Oncology Program at Shands can see patients on referral for the development of a treatment plan in conjunction with their primary physician, for a second opinion, or for ongoing management by UF Shands Cancer Center physicians. Results of clinical evaluations and treatment recommendations are provided to referring physicians or to patients and their families.
Appointments - Please call (352) 265-0725, Extension 8-7670, Monday-Friday, 8:30 A.M. - 4:30 P.M., to set up an appointment for clinical evaluation or second opinion.
Referral Checklist
The following material should be provided to the Neuro-Oncology Program at Shands by FAX or FedEx (or other expedited carrier) at least 2 working days before the patient's scheduled appointment.
- A synopsis of clinical history, operations, radiation and chemotherapy, current state of patient and proposed studies or therapy.
- A list of specific questions being posed and the therapies being considered.
- The name, address, and telephone number of the patient (for confirmation of appointment).
- The name, address, telephone and fax numbers of the referring physician.
Patients should bring the following material (if available) with them to clinic on the day of their appointment:
- Copies of the actual head or spine MRI or CT scans (not reports) performed at the time of diagnosis to include the most recent studies.
- The original glass pathologic slides, and 5 unstained section along with a copy of the previous pathologic report.

Copies of relevant medical records (hospital admissions, operative reports, specialty consultations, treatment records, etc.)
