I am feeling a little overwhelmed and need to talk to someone.
Where can I go?
Contact Dr. Vidaurreta, at the Office of Counseling and Development, Room CG-82, or call 392-5482. Dr. Duff (at 273-7673) and your advisor/mentor are also available for counseling.
What do I do if I need a transcript?
Beginning Monday, February 25, 2008, the University of Florida will begin charging for official transcripts. The costs will be $6.00 per transcript for current students and $12.00 per transcript for students who are not enrolled.
To order a transcript, go to ISIS, open "my record," select "transcripts" and follow the prompts. Online orders are paid through a secure credit card transaction using VISA, MasterCard, American Express, Diner's Club, or a debit card affiliated with a major credit card company. A GatorLink account is necessary to complete the transaction.
Instructions will help you complete all the necessary steps even if you do not have a GatorLink account.
Current and former students with valid GatorLink accounts can view and print a free unofficial copy of a transcript from the ISIS Web site.
Who completes my Loan Deferment forms?
Please bring all Loan Deferment paperwork to the Office of Student Affairs (Room M125-MSB) to be completed. We will mail it upon completion. Otherwise, please indicate whether you would like to pick it up for have it put in your PO Box. Please do not fill in the dates. We will complete the dates. We can only complete the form each academic year. Your prior school may already be registered in the National Student Loan Data System and such manual deferment forms may not be necessary after your first term in Medical School.
Where do I get a letter to verify that I am currently enrolled?
If you are in need of a letter verifying your enrollment here at UF, please come to the Office of Student Affairs (Room M125-MSB) and complete a Request for Letter form. If you provide a mailing address, we will put it in the mail upon completion. Otherwise, please indicate whether you would like to pick it up or have it put in your PO Box.
What if my ID Badge is lost or stolen?
You must come by the Office of Student Affairs (Room M125-MSB) to complete a form to be taken to the Biomedical Media Services in order to purchase a replacement ID badge. There will be a cost for a replacement ID badge. Contact OSA (352) 273-7971 before going to BMS.
What do I do if I've misplaced my mailbox key?
You must come by the Office of Student Affairs (Room M125-MSB) and we will request a new mailbox key for you. Please be aware that the Mailroom may begin to charge a fee for a new key. Please keep track of your PO Box key at all times.
Where can I pay my fees?
Fees can be paid in a number of ways. You may pay online through ISIS. You may take the payment to Criser Hall and pay by check (for credit card payments, please contact UFS at (352) 392-0181) to the cashier at University Financial Services. There is also a UFS Night Deposit box in the courtyard of Criser Hall. Or, you may write a check and mail it to:

Where do I go to change my name?
Request a "Name Change Form" from the staff in Student Affairs Office, Room M-125. It must be turned in to main campus. Please do not submit xeroxed forms. Once it is changed according to directions, please let Student Affairs office know at (352) 273-7971.
I've moved where can I change my address or phone number?
There are actually 2 steps to changing your address.
- To change your address within the University system, go into ISIS and complete the information on line.
- Address change forms are available in Student Affairs Office in Room M-125. Complete the form and leave in the box on the wall.. This information will be used to update College of Medicine files.
I lost my class schedule, where may I obtain a new one?
Contact the Office of Medical Education in Room CG-78, or call them at 392-3690
I am sick/have an emergency and I'm unable to attend class today, what do I do?
Call Student Affairs at (352) 273-7971 or the Office of Medical Education at 392-3690 and leave word regarding your situation.
I know in advance that I will need to miss class---for any reason.
Contact the course director in advance and make arrangements for your absence. You must also notify Student Affairs, (352) 273-7971.
I am sick and need to see a doctor.
If you have health insurance through the University, your first contact must be the Student Health Care Center. If you have private insurance you may either see a physician at the SHCC or a private physician.
I am concerned about a friend.
Call Dr. Vidaurreta at 392-5482 or visit her in Room CG-82.
I am having academic problems. I don't know where to start.

My grade report and the grades on my transcript do not agree. What do I do?

My parents would like to know how to contact me in case of an emergency.
Have them call the Office of Student Affairs at (352) 273-7971. You may also update your parents' address in our office as well.
I have a financial flag/hold on my record. What do I do?
Contact University Financial Services at 392-0181 to determine the reason for the financial hold. For a non-financial flag/hold, contact Amelia Jaworski in Office of Student Affairs, (352) 273-7971.
