This form is for the Student Affairs Office so we may have a current record of your address and contact information in case of emergency. When you change your address after you arrive here at the College of Medicine (COM), we require you to not only change your address with our office, but you will also have to change it on ISIS as well.

Is it extremely important that you keep the Student Affairs office informed of any address change you might have between now and Orientation. We may send you important forms and questionnaires and we want to make sure you receive everything you need. If you change your address please complete the Address Change form immediately! If you should change your name, please contact the Student Affairs office so that the proper name change forms can be sent to you! - Immunization Verification Form (PDF)
If you have any questions regarding immunizations, please contact Student Health at 392-1161, extension 4312. It is important that you identify yourself as a medical student. Medical students have stricter requirements than some other UF students. The following immunizations are required by the College of Medicine in addition to those required of all UF students:
- proof of a positive varicella titer or documentation of Varivax
- first in the Hepatitis B Series
- PPD immunization must be within the last 6 months of entrance to medical school.
If you have tested positive, you must bring in a "Review of Symptoms" form from Student Health Services. Additional Immunization Information (PDF)
- Health Agreement (PDF)
This form is signed only once. It contains a list of agreements that all medical students adhere to in order to be in good standing with the College of Medicine.
- Health Insurance Verification Form (PDF)
This must be completed each year by your Health Insurance Provider
- Disability Insurance (PDF)
The College of Medicine requires medical students to carry Disability Insurance. We are able to obtain this insurance at a nominal cost to you.
We are pleased to announce a Global Emergency Services program is now available to AMA-sponsored Med Plus Advantage participants at no additional cost. Please view the program brochure detailing the services provided, along with a PDF containing a cut-out card the students/residents should keep in their wallet at all times in case of an emergency. We are excited about this new offering as it provides an array of services when they are traveling 100 miles or more from their home/school residence. Services such as medical repatriation, emergency medical evacuation, and medical consultation and referral are just a few of the services provided through Assist America's program. Should an emergency occur while traveling, the students/residents simply call the number shown on the card and Assist America will make the necessary arrangements.
- Matriculating Student Questionnaire
This is a questionnaire that must be completed on line to the AAMC. It evaluates the medical school application process. The Questionnaire will be available July 1st and must be completed by September 11, 2009.
- Fee for Identification Badge
All faculty, staff and students at the University of Florida are required to have university picture ID badges. These badges are necessary for everything – checking out books at the library, using the recreation facilities, tickets to sporting events, etc. Students in the University of Florida Health Science Center are required to wear their photo identification badge at all times within the Health Center. ID badges will be made on the afternoon of Monday, August 10, 2009. This time has been especially reserved for medical students and you must have your badge made at this time. (Please be prepared to present photo ID or driver’s license.)
The cost is $16.25 and must be paid prior to Orientation. Please mail a check for this amount to the Student Affairs office by July 1, 2008 made payable to "University of Florida". Send your check to: Student Affairs Office; PO Box 100216; Gainesville, FL 32610-0216.
Note: If you attended UF as an undergraduate, you must turn in your old ID badge and purchase a new one. - Getting To Know You (PDF)
In order to help you become well acquainted with your classmates, the Student Affairs office will compile a class directory, "Getting To Know You", with pictures and miscellaneous information about each student. Please complete the Getting To Know You form and attach a passport-sized photo of your face (2x2 or 3x3). Please return the form to our office by July 1 in order to have your information included. The directory will be part of your orientation packet. Many faculty and staff request a copy of the "Getting To Know You" directory so that they may become acquainted with the new students as well. - Faculty Dinner (and Lunch) Preference/Info Sheet (PDF)
For many years, faculty members in the College of Medicine have volunteered to host small dinners in their homes for new medical students. This event is probably the favorite of all orientation activities and the one event you will not want to miss.

- Tuition
You will be notified as soon as we know the exact amount of tuition and fees for 2009-2010. As a medical student you will pay your yearly tuition in two equal installments: one half in the fall and the other half in the spring. The summer of 2009 you will only be required to pay a $30 lab fee. (You must remember to do this.) If you have applied for financial aid, and the amount of tuition changes, your budget will be adjusted accordingly. Please Note: The University of Florida does not send bills for tuition/fees. It is the responsibility of each student to assess fees correctly and pay on time. The Office of Student Affairs will keep you posted on due dates.
