Letter of Offer Checklist
In accordance with the rules of the Florida Board of Education, the Florida Administrative Code and College of Medicine guidelines, certain items must be included in the Letter of Offer. (See list below.)

Further information pertaining to each item may be found in the Explanation of Checklist for New Appointments, Section 15a.

_____ 11. CLINICAL FACULTY STATEMENTS  for all faculty include the following:
  • Patient Records Statement
  • License and Privileges Statement
  • Medicaid Statement
  • Medicaid Credit Balance Letter attachment (Must be included with appointment paperwork)
  • _____ 12. HEALTH ASSESSMENT STATEMENT
    _____ 13. OUTSIDE EMPLOYMENT STATEMENT
    _____ 14. COMPLIANCE STATEMENT (Must be included in all letters of offer)
    _____ 15. HIPAA STATEMENTS
    _____ 16. RETIREMENT CONTRIBUTION STATEMENT(not required for Post Docs)
    _____ 17. IMMIGRATION STATEMENT (if applicable)**Please Note: If appointee is on H-1 visa, a separate addendum will be required with the letter of offer. Please refer to the letter of offer instructions for more detailed information.
