Transfer of UFF Donor Funds to UF Through DSR
In order for DSR to establish the initial UF companion account for purposes of receiving disbursements from the University of Florida Foundation (UFF) donor funds, the following will need to be provided:
(1) A completed DSR-1
Please make sure you include the UFF Fund # & Title
(2) UFF Disbursement Request Form (UFF-D)
Please make sure you include UFF Fund Number,a clear statement describing the purpose and use of funds, and Fund Administrator information.
(3) Transfer to UF Request Form (UFF-U)
Please make sure you include UFF Fund Number and Title, Project Period (if indefinite, state indefinite), a clear statement describing the purpose and use of funds, a clear statement indicating donor restrictions, (if none, state none), a clear statement indicating reporting requirements (if none, state none), and Fund Administrator information.
Please complete all items listed above and submit the original DSR-1 and a copy of the UFF-D and UFF-U to ORA for College of Medicine review and approval. The original UFF-U and UFF-D should be sent to UFF. Incomplete items will be returned.
